Florida Community Health Centers (FCHC) is pleased to announce the renewal of Patient-Centered Medical Home (PCMH) certification for ten of its healthcare centers. The certified locations include Clewiston, Lakeshore Medical, Fort Pierce, Indiantown, Moore Haven, Pahokee, Stuart, Darwin Square, Dr. Fred Brown Children’s Health Center, and Port St. Lucie Pediatrics.
The PCMH model is designed to place patients at the center of their healthcare, fostering strong, collaborative relationships between individuals and their care teams. This approach promotes coordinated, comprehensive, and accessible care that is tailored to each patient’s unique needs.
Practices recognized as PCMH-certified have demonstrated measurable improvements in the quality of care, patient satisfaction, and staff engagement while also helping to reduce overall healthcare costs. Certification is granted by the National Committee for Quality Assurance (NCQA), a leading authority that upholds high standards in patient-centered care and continuous quality improvement.
