The certified centers include Clewiston, Lakeshore Medical, Fort Pierce, Indiantown, Moore Haven, Pahokee, Stuart, Darwin Square, Dr. Fred Brown Children’s Health Center, and Port St. Lucie Pediatrics. PCMH is a care model that prioritizes patients’ needs and fosters strong relationships between patients and their clinical care teams.
PCMH-certified practices have consistently demonstrated improvements in quality of care, patient experience, and staff satisfaction, all while reducing healthcare costs. The National Committee for Quality Assurance (NCQA) recognizes centers that adhere to PCMH principles, signifying a commitment to continuous quality improvement and patient-centered care.
