We are proud to announce that Florida Community Health Centers Inc. (FCHC) has received Community Health Quality Recognition (CHQR) badges from the Health Resources and Services Administration (HRSA) in acknowledgment of our exceptional performance in Quality and Health Information Technology during the most recent UDS reporting period. These recognitions highlight our ongoing commitment to delivering high-quality, patient-centered care and improving health outcomes throughout the communities we serve.
FCHC was awarded the National Diabetes Health Badge for exceeding national targets in key clinical measures, including a 94% rate for Adult BMI Screening and Follow-Up Plan, and a 93% rate for Child & Adolescent Weight Assessment, Nutrition, and Physical Activity Counseling. In addition, we received the Behavioral Health Badge for achieving a 95% rate in Depression Screening and Follow-Up Plan (ages 12+), and a 26% rate for Depression Remission at 12 Months. We were also honored with the Advancing Health Information Technology (HIT) for Quality Badge, recognizing our exceptional use of telehealth services and our efforts in actively engaging patients through innovative health information technology.
