Florida Community Health Centers Inc. (FCHC) is proud to announce that Stephanie Taylor, PharmD, CPh, RPh, Director of Pharmacy, was a featured presenter at the recent National Center for Equitable Care for Elders webinar. The webinar, titled “Navigating Polypharmacy Management in Older Adults,” focused on optimizing medication plans for community-dwelling older adults, especially those with multiple chronic conditions.
During the presentation, Stephanie Taylor co-presented with Dr. Andrea Schwarz, a Geriatrics Fellow of the American Geriatrics Society. Together, they explored person-centered approaches for shared decision-making in the deprescribing process, emphasizing the importance of tailored medication plans for older adults.
Stephanie Taylor’s expertise and insights were sought to shed light on the crucial role of pharmacists in managing polypharmacy and to provide guidance on how health centers can best prepare for addressing the unique needs of an aging population. Her perspective on navigating complex medication regimens and promoting patient-centric care was well-received by webinar participants.
