As a primary care provider and medical home, we strive to provide a variety of services for the convenience of our patients. For those services we do not provide, FCHC assists its patients in coordinating care across multiple settings, including specialty offices and hospitals. Our role is to understand and explain to our patients the overall picture of their health care and to assist with eliminating in gaps service recommended for becoming and remaining healthy. In order to do so, FCHC strives to maintain complete and accurate medical records. We make every attempt to obtain all relevant information from other Clinicians involved in the patient care so that we can assist with keeping our patients informed. We ask for our patients help in this process by providing complete and accurate medical information. This includes their medical history as well as information about medical care received elsewhere.

We are available to assist our patients 24 hours a day, 7 days a week. We have same day appointments available and our patients may walk in if needed. We encourage our patients to call us if they have a medical concern or are trying to decide if they should go to an urgent care center or ER. We always urge patients to dial 911 for true emergencies such as (but not limited to) continuous bleeding, suspected poisoning, active seizures, immediate problems breathing, suspected heart attack or stroke, or large cuts or burns.
