Job Summary:         
The Referral Specialist is responsible for coordinating and completing referrals ordered by FCHC providers, acquiring and verifying third party information for referrals, obtaining and arranging referral appointments. Tracking and maintaining reports in the i2i system when referrals are completed, no-showed or refused and maintain referral follow-ups.

Referral Specialist – Primary:
1. Coordinate all aspects of completing referrals ordered by FCHC providers (i.e., obtain pre-op authorizations and insurance authorizations.
2. Contact patients once authorization obtained.
3. Arrange referral appointments ensuring proper paperwork is faxed to a specialist for appointment per orders (i.e., labs, x-rays, comp notes).
4. Track referral “no shows”, in the medical record and bring to the attention of the ordering physician.
5. Maintain communication with the provider and patient regarding the most appropriate specialist to refer to in each case.
6. Maintain specialty provider list including payor sources they accept.
7. Input referrals into the i2i tracking system and generate referral form.
8. Make appropriate notations in the medical record and bring to the attention of the ordering Provider and/or Center Clinical Director any problems regarding obtaining referrals.

Minimum Qualifications:

  1. High school diploma or
  2. Proof of satisfactory completion of an approved medical assistant program or nursing assistant program; or

One (1) year of experience in a medical-related field.

  1. Registered and Certified Medical Assistants must maintain current RMA/CMA
  2. Current CPR

History

Since 1976, Florida Community Health Centers Inc. has been a leader in providing health care services in Florida. Comprehensive primary health care services are provided through a network of health centers surrounding Lake Okeechobee and throughout Florida’s Treasure Coast.  

FCHC has health center locations in Clewiston, Fort Pierce, Indiantown, Moore Haven, Okeechobee,  Pahokee, Port St. Lucie, and Stuart with FCHC’s Corporate Office centrally located in West Palm Beach, Florida. There are over 400 employees, in which 100 are clinicians. FCHC has multiple staff members who speak Spanish and Creole, and translation is available for other languages as needed. FCHC has created a “one-stop shop” for patients providing pediatric, adult primary, dental, OB/GYN and pharmacy services, with an option to have prescriptions delivered by mail.Our patients benefit from a “medical home” model, where they may access and benefit from our extensive array of health care clinicians and services at each location. Utilizing a sliding fee scale (SFS), based on annual federal poverty limits (FPL), we are able to determine the amount of financial responsibility each patient carries regarding their health care services. Patients who are at or below 100% of FPL have their health care needs provided by FCHC at a nominal charge.

FCHC is a private, non-profit, tax exempt 501(c)3, consumer directed corporation designated as a Federally Qualified Health Center (FQHC), and accredited by The Joint Commission (JC), as well as recognized as a Level 3 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).
