ADA:

Full time position. Job duties may require holding/lifting patients to or onto an exam table. Ability to sit and/or stand periodically during the workday. Computer data entry a portion of job duty.

OSHA:

Must adhere to universal precautions, to include bloodborne pathogen protection, at all times.

Primary Duties

  1. Evaluate patient’s chronic problems thoroughly and manages appropriately, as demonstrated by performance on Clinician dashboard, Quality Measures Performance, and other applicable reports (peer reviews, any focused reviews, and patient satisfaction surveys).
  2. Evaluates patient’s acute problems thoroughly and manages appropriately (including medications, imaging, and labs).
  3. Utilizes Quality Measures Tab to provide health maintenance services at recommended intervals.
  4. Consistently closes encounters within 24 hours to generate a visit summary for patient flow and continuity of care.
  5. Uses consultants appropriately and cost-effectively as evidenced by referral frequency and appropriateness reflected in the referral department and other available reports. Does basic appropriate workup before referral.
  6. Demonstrates sound judgment and decision-making skills in day-to-day practice activities (e.g., triage appropriately, patient flow management, etc., etc.)
  7. Updates Problems, Medications, and Vaccine lists appropriately.
  8.  Completes ROS, anticipatory guidance, preventive and screening components.
  9. Adheres to accepted evidence-based standards of care, as applicable.
  10. Encounters are completed in a timely manner and accurately.
  11. Charges are submitted by the end of the day (or end of hospitalization, if applicable).
  12. Coordinates and leads daily team huddles.
  13. Medication lists are updated and reconciled per policy.
  14. Quality Measures Performance Report demonstrates performance within acceptable ranges.
  15. Clinician Utilization Review Grid demonstrates performance is within acceptable ranges.

Secondary Duties:

1. Responds professionally to change (i.e., schedule, work assignments).

2.  Attends continuing education events and shares a summary with peers.

3. Appropriately uses auxiliary departments (HBN, Care Coordination).

4.  Is flexible in meeting center needs outside regular working hours, (i.e., health fair, community lectures, etc

5.  Responds appropriately to adversity or stress.

6.   Renews license, privileging, credentialing and DEA number (if applicable) in a timely fashion.

7.  Coordinates and leads team activities consistent with the function of PCMH.

Duties for Obstetrical/Gynecological Physicians Only

 Reads and interprets obstetrical ultrasounds and biophysical profiles.

Minimum Qualifications:

1.  Current and active State of Florida, Department of Health license (MD, DO, DDS, DMD, RDH).

2.  Current DEA certification.

History

Since 1976, Florida Community Health Centers Inc. has been a leader in providing health care services in Florida. Comprehensive primary health care services are provided through a network of health centers surrounding Lake Okeechobee and throughout Florida’s Treasure Coast.  

FCHC has health center locations in Clewiston, Fort Pierce, Indiantown, Moore Haven, Okeechobee,  Pahokee, Port St. Lucie, and Stuart with FCHC’s Corporate Office centrally located in West Palm Beach, Florida. There are over 400 employees, in which 100 are clinicians. FCHC has multiple staff members who speak Spanish and Creole, and translation is available for other languages as needed. FCHC has created a “one-stop shop” for patients providing pediatric, adult primary, dental, OB/GYN and pharmacy services, with an option to have prescriptions delivered by mail.Our patients benefit from a “medical home” model, where they may access and benefit from our extensive array of health care clinicians and services at each location. FCHC is starting the process to expand our behavioral health and susbstance use disorder services.

Utilizing a sliding fee scale (SFS), based on annual federal poverty limits (FPL), we are able to determine the amount of financial responsibility each patient carries regarding their health care services. Patients who are at or below 100% of FPL have their health care needs provided by FCHC at a nominal charge.

FCHC is a private, non-profit, tax exempt 501(c)3, consumer directed corporation designated as a Federally Qualified Health Center (FQHC), and accredited by The Joint Commission (JC), as well as recognized as a Level 3 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).
