Location: Fort Pierce Center

Primary Duties:
1. Ensure that all required contractual items are in place, (i.e., posters in lobby and on doors, as well as adequate brochures, CMS Certificates of CAC). Monitor each center for compliance quarterly.
2. Supervise staff assigned to the following programs: Health Benefits Coordinators and Outreach Coordinators.
3. Monitor and review data to ensure conformity to organizational standards and protocols established across programs.
4. Communicate with other FCHC supervisors as needed.
5. Responsible for ensuring training for new Health Benefits Coordinator and Outreach Coordinator staff is scheduled and completed. Conduct employee introductory and annual employee evaluations with input from local supervisors.
6. Maintain documentation and data collection in accordance with FCHC and other specified agency guidelines. Conduct and/or participate in program evaluation as directed.
7. Supervise Health Benefits Coordinator and Outreach Coordinator positions to ensure compliance with program guidelines and FCHC policies. Provide comprehensive monthly report to the Director of Special Services by the 10th working day of the month.

Secondary Duties:
1. Provide second party review of chart audits to monitor compliance.
2. Monitor contracts involving Health Benefits Coordinator and Outreach Coordinator staff to ensure program requirements are met.
3. Travel to other FCHC sites, meetings, trainings and other FCHC activities when required.
4. Assist in activities, and participate in health fairs with Health Benefits and Outreach Coordinators.

Minimum Qualifications:
1. Associate’s Degree, with three years of experience where the major responsibility was in any one of the following categories: public health, community health center, economic services, counseling, investigating, medical billing/collections, and/or processing insurance claims. High School Diploma or its equivalent with five years of experience, in the above mentioned categories may be substituted for the required education.
2. Valid Florida Driver’s License and have reliable/accessible transportation.
3. Bilingual Spanish/English preferred
4. Must complete required CMS Certified Application Counselor (CAC) Training and receive passing score.
5. Must maintain Certification as CAC, as a condition of employment.

Knowledge, Skills, and Abilities:
1. Must be able to assist in the application of various public assistance programs, entering very sensitive information into a variety of computer systems in a fast paced environment.
2. Expected to handle a high volume of work with strict deadlines.
3. Must be able to prioritize work, and be flexible to changes at any moment.
4. Must be able to work with clients who may be unable to read, write and are unable to operate a computer.
5. Must abide by the policies, procedures, laws, rules and regulations of Florida Community Health Centers, Inc, The Department of Children and Families, Florida Healthy Kids Corporation (KidCare) and any other social program for which an application is made.
6. Must be prepared to report suspected child or elderly abuse as well as suspected fraud to the appropriate authorities.
7. Must have knowledge of community resources.
8. Must be computer literate and have experience within Microsoft Office.
9. NextGen software experience preferred.
10. Be culturally sensitive.

History

Since 1976, Florida Community Health Centers Inc. has been a leader in providing health care services in Florida. Comprehensive primary health care services are provided through a network of health centers surrounding Lake Okeechobee and throughout Florida’s Treasure Coast.  

FCHC has health center locations in Clewiston, Fort Pierce, Indiantown, Moore Haven, Okeechobee,  Pahokee, Port St. Lucie, and Stuart with FCHC’s Corporate Office centrally located in West Palm Beach, Florida. There are over 400 employees, in which 100 are clinicians. FCHC has multiple staff members who speak Spanish and Creole, and translation is available for other languages as needed. FCHC has created a “one-stop shop” for patients providing pediatric, adult primary, dental, OB/GYN and pharmacy services, with an option to have prescriptions delivered by mail.Our patients benefit from a “medical home” model, where they may access and benefit from our extensive array of health care clinicians and services at each location. FCHC is starting the process to expand our behavioral health and susbstance use disorder services.

Utilizing a sliding fee scale (SFS), based on annual federal poverty limits (FPL), we are able to determine the amount of financial responsibility each patient carries regarding their health care services. Patients who are at or below 100% of FPL have their health care needs provided by FCHC at a nominal charge.

FCHC is a private, non-profit, tax exempt 501(c)3, consumer directed corporation designated as a Federally Qualified Health Center (FQHC), and accredited by The Joint Commission (JC), as well as recognized as a Level 3 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).
