About Us




Since 1976, Florida Community Health Centers Inc. has been a leader in providing health care services in Florida. Comprehensive primary health care services are provided through a network of health centers surrounding Lake Okeechobee and throughout Florida’s Treasure Coast.  

FCHC has health center locations in Clewiston, Fort Pierce, Indiantown, Moore Haven, Okeechobee,  Pahokee, Port St. Lucie, and Stuart with FCHC’s Corporate Office centrally located in West Palm Beach, Florida. There are over 400 employees, in which 100 are clinicians. FCHC has multiple staff members who speak Spanish and Creole, and translation is available for other languages as needed. FCHC has created a “one-stop shop” for patients providing pediatric, adult primary, dental, OB/GYN and pharmacy services, with an option to have prescriptions delivered by mail.Our patients benefit from a “medical home” model, where they may access and benefit from our extensive array of health care clinicians and services at each location. FCHC is starting the process to expand our behavioral health and susbstance use disorder services.

Utilizing a sliding fee scale (SFS), based on annual federal poverty limits (FPL), we are able to determine the amount of financial responsibility each patient carries regarding their health care services. Patients who are at or below 100% of FPL have their health care needs provided by FCHC at a nominal charge.

FCHC is a private, non-profit, tax exempt 501(c)3, consumer directed corporation designated as a Federally Qualified Health Center (FQHC), and accredited by The Joint Commission (JC), as well as recognized as a Level 3 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA).

Gold_Joint CommissionFCHC is accredited by The Joint Commission. In order to receive and maintain Joint Commission Accreditation, sites must successfully undergo an intensive on-site review. Surveyors with expertise in the clinical, financial and administrative aspects of health care thoroughly examine the organization’s policies and procedures as relates to providing quality, evidenced based care with a focus on patient safety and prevention of negative outcomes. Surveyors also visit practice sites and interview a variety of staff persons to assess staff competency and the consistency of practice in accordance with the company’s written policies.

The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.

As a primary care provider and medical home, we strive to provide a variety of services for the convenience of our patients. For those services we do not provide, FCHC assists its patients in coordinating care across multiple settings, including specialty offices and hospitals. Our role is to understand and explain to our patients the overall picture of their health care and to assist with eliminating in gaps service recommended for becoming and remaining healthy. In order to do so, FCHC strives to maintain complete and accurate medical records. We make every attempt to obtain all relevant information from other Clinicians involved in patient care so that we can assist with keeping our patients informed. We ask for our patients to help in this process by providing complete and accurate medical information. This includes their medical history as well as information about medical care received elsewhere.

We are available to assist our patients 24 hours a day, 7 days a week. We have same day appointments available and our patients may walk in if needed. We encourage our patients to call us if they have a medical concern or are trying to decide if they should go to an urgent care center or ER. We always urge patients to dial 911 for true emergencies such as (but not limited to) continuous bleeding, suspected poisoning, active seizures, immediate problems breathing, suspected heart attack or stroke, or large cuts or burns.
